Initial Appointment​

• Submit an appointment request to reserve your time. Afterward, please check your email for intake forms to complete. Your appointment will be confirmed once the forms are submitted. Please complete intake forms 7 days before your first visit or the request will be canceled so we can offer the slot to other patients. If you book within 7 days, forms are due 3 days before the visit. If you book within 3 days, please complete forms immediately; appointments booked <72 hours may be canceled if forms aren’t received by 5:00 pm the business day before.
