• You’ll receive an email with required intake forms, expect to receive the email within 24 business hours. Your appointment is confirmed once all forms are completed.​

• To keep your appointment, intake forms must be completed within 72 hours of receipt. If your appointment is scheduled within 72 hours, please complete forms immediately.​

• Appointment times are reserved specifically for you. Missed or late-canceled initial appointments are subject to a fee equal to the out-of-pocket cost of the appointment.

• To avoid a missed appointment charge, please cancel or reschedule with at least 24 business hours’ notice. This policy helps ensure availability for other patients seeking care.
